Salter HealthCare is pleased to announce Aberjona Rehabilitation and Nursing Center is now a preferred provider in the Atrius Health network.
Owned and operated by Salter HealthCare, Aberjona Rehabilitation and Nursing Center is a 123-bed facility located at 184 Swanton Street in Winchester providing short-term rehabilitation, skilled nursing services andlong-term care. Aberjona’s state-of-the-art rehabilitation wing features a separate entrance and dining room for patients requiring short-term stays. The facility’s intensive short-term rehabilitation program is designed to help patients regain their ability to live independently following surgery or illness. Physical, occupational, speech and respiratory therapies are available daily and the Transitions Suite features a home-like kitchen and bathroom model that allows patients to practice activities of daily living before returning home.
In 2012, Atrius Health created a preferred network of skilled nursing facilities to collaborate more closely with to improve the coordination of care of patients requiring SNF services. Their goals were to improve communication and sharing of information between the SNFs and primary care physicians, to decrease the length of a patient stay in a SNF (when medically appropriate) and to reduce the number of hospital readmissions after a patient has been discharged from a SNF and sent home.
To be considered a preferred provider, skilled nursing facilities undergo an extensive evaluation process and must meet specific criteria that includes location of care, facility rating, facility attributes, willingness to collaborate and value to patient. Preferred provider SNFs are assessed on an ongoing basis and must continue to meet Atrius Health’s established standards.

Atrius Health physicians and other providers accept most major health insurance plans, including Aetna, Blue Cross Blue Shield of Massachusetts, Fallon Community Health Plan, Harvard Pilgrim Health Care, Neighborhood Health Plan, Tufts Health Plan, and Tufts Medicare Preferred.
