  • Access free language services for LEP individuals (e.g., qualified interpreters, translated materials)
  • File a grievance with Stellar Health Group and/or a complaint with appropriate government agencies

4. Grievance Procedure

You may submit a grievance in person, by phone, by email, or in writing. Please include your name, contact information, a description of the concern, the date(s) involved, and any requested resolution. We will conduct an impartial investigation and issue a written decision within a reasonable timeframe.

Non-Retaliation: We prohibit retaliation against any person for filing a grievance or participating in an investigation.
